How to Request a Cardiology Appointment with My Heart Spark P.C.

Your next step toward cardiovascular care begins with choosing the service that fits your request. My Heart Spark P.C. currently directs public appointment requests to its established SimplePractice patient request system.

Choose the care pathway that fits your request

Short cardiology visit — 15 minutes

For visitors seeking the shorter cardiology visit option, select the 15 minute cardiology service on the appointment request page. The public service list currently displays a $50 price for this option. Review current terms within the request system.

Extended cardiology visit — 45 minutes

For a longer cardiology discussion, select the extended 45 minute visit option. The public service list currently displays a $150 price. Current charges and available times are determined in the request system.

Second opinion and records review

The service menu also provides a second opinion medical records review pathway labeled as a service without a live visit. Review its current description and instructions in the patient request system to understand the scope and information required.

POTS information and resource pathway

The public service menu separately lists a POTS care resource package and an information option. These are identified within the existing service chooser. Review the description to select the educational resource or information pathway you intend.

How to submit a request

  1. Open the existing My Heart Spark P.C. appointment request page.

  2. Read the available service descriptions, durations, and current prices.

  3. Select your intended appointment, records review, or resource option.

  4. Follow the secure patient request process for the next available steps and any information requested.

Before you begin

Consider having a list of your current medications, relevant cardiovascular questions, and prior medical records available as appropriate to your request. Follow instructions in the secure patient system for any records or personal health information.
