What to prepare


If available, bring recent home blood pressure readings with dates and times, a list of prescribed and over the counter medications, relevant laboratory results, and prior diagnoses. Tell the clinical team about symptoms, pregnancy, kidney disease, or prior cardiovascular events where relevant. Clinical records should be exchanged through a practice approved secure channel.


How to request care
